Surgical Treatment of Sarcomas
OSTEOCHONDROMA
OF THE FEMORAL NECK

Osteochondromas of the femoral neck represent rare intraarticular
lesions. The difficulty to obtain adequate surgical access especially
when located at the inferior and posterior femoral neck has been expressed
already in the literature. The authors have approached these lesions with
a new, versatile approach which offers the advantage of complete tumor
visualization, circumferential access to the femoral neck combined with
the possibility of entire hip joint inspection and intraoperative control
of adequate tumor resection. The surgical approach includes a digastric
trochanteric osteotomy and subluxation or even dislocation of the femoral
head thereby respecting the crucial blood supply to the femoral head.
This approach has been used in 4 patients with osteochondroma of the femoral
neck presenting with pain, restricted range of motion and limping. Femoro-acetabular
impingement of the bulky osteochondroma against the acetabular rim could
be verified in all cases. In 2 patients labral lesions were found at the
impingement site. All 4 patients had complete or significant relief of
symptoms at a mean follow-up of 34 months (18 – 48 months) without signs
of progressive osteoarthritis or avascular necrosis.
